As the Housing Case Manager, you will play a pivotal role in individuals' lives, making a significant impact through:
- Based on the completed assessment, determine the number of times per month to meet with each household. Each household should be contacted either in person or by phone at least once every other week.
- Assist in eliminating barriers to achieving goals outlined in the service plan, including financial assistance, if necessary, and transportation assistance to ensure households are receiving needed area services.
- Provide direct services to households who are referred to Catholic Charities Housing Program, to include advocacy, coordinating services, referrals, transportation, budgeting, employment readiness, financial stability, accessing public benefits and applying for long‐term financial support, etc. Services may be offered in the individual’s or family’s home, in the community, at another service provider’s office, or at Catholic Charities offices.
- Coordinate within Catholic Charities and with other agencies to provide services to clients.
- Assist with monitoring the Day Resource Room.
- Maintain up‐to‐date case files and document all case management activities, including assessment, service
Housing Case Manager - Minimum Salary: $22.00 per Hour
Housing Case Manager - Maximum Salary: $22.00 per Hour
Housing Case Manager - Schedule: Monday, Tuesday, Wednesday, & Friday 8:30AM to 4:30PM; Thursday 8:30AM to 6:30PM
Qualifications:
- Associates Degree in Human Services or related field.
- 1 to 3 years working in a Human Service industry.
