Role Overview
The Director of Community Care Coordination for Affordable Housing provides strategic and operational leadership for service coordination across 19 affordable and supportive housing communities serving older adults throughout Chicago and surrounding suburbs. The role ensures residents have access to high-quality, person-centered services that promote housing stability, well-being, and the ability to age in place with dignity.


Job Responsibilities
  • Oversee care coordination services across 19 affordable and supportive housing communities; design and implement integrated service models that promote housing stability, aging in place, behavioral health, and overall wellness; standardize service coordination policies, procedures, workflows, and performance metrics across all locations; monitor caseloads, service utilization, staffing capacity, and operational effectiveness to meet resident needs; and ensure compliance with regulatory requirements, funding guidelines, and organizational policies.
  • Identify and implement opportunities for program growth, innovation, and service expansion; utilize data and outcome measures to inform strategic planning and continuous quality improvement; and evaluate program effectiveness through analysis of resident outcome indicators.
  • Establish and maintain strategic partnerships with healthcare providers, behavioral health organizations, managed care organizations, government agencies, and community providers; and represent the organization in coalitions, advisory groups, and cross-sector initiatives to strengthen collaboration and advance organizational objectives.
  • Partner with Property Management, Housing, Benefits, Finance, People and Culture, and other departments to ensure coordinated service delivery; and collaborate with organizational leaders to align service coordination strategies with organizational goals and priorities.
  • Lead and develop the Community Care Coordination team by setting clear goals and expectations, monitoring performance, and providing regular feedback and guidance. Inspire team excellence by modeling accountability, collaboration, and solution-oriented thinking, and ensure team members are supported, empowered, and high-performing while fostering a positive, purpose-driven culture.
  • Develop staff capability through ongoing training, mentorship, and professional development opportunities. Cultivate a culture of continuous learning, feedback, and improvement to strengthen team engagement, retention, and impact.

PLEASE NOTE: Essential functions include all other duties and responsibilities as assigned.
  • Kneel and move from sitting, bending, kneeling, or standing multiple times a day

Education and Experience Requirements:
Minimum
  • Bachelor's Degree in Arts/Sciences (BA/BS) in a related field of study 1-3 years relevant experience
  • Strong knowledge of service coordination, supportive housing, behavioral health, and resident services, with the ability to lead programs, ensure compliance, analyze outcomes, build strategic partnerships, and drive operational excellence.
  • Excellent leadership, communication, collaboration, and problem-solving skills are required.
Preferred
  • Master's degree in Arts/Sciences (MA/MS) in a related field of study 3-5 years relevant experience
  • Experience in affordable or supportive housing, healthcare, behavioral health, aging services, or community-based programs, with demonstrated success leading multi-site service delivery, implementing integrated care models, and fostering cross-sector partnerships that enhance resident outcomes and organizational impact.

Certifications and Licensure
  • Minimum: Obtain Mental Health First Aid certification within 6 months of hire. Obtain Professional Service Coordinator (PSC) certification or comparable affordable housing resident services credential within 18 months of hire.
  • Preferred: LSW, LCSW, Professional Service Coordinator (PSC), or equivalent resident services certification; training in trauma-informed care, crisis intervention, motivational interviewing, or aging services.
