What are the responsibilities and job description for the St. Louis Area Case Manager position at Catholic Charities of St. Louis?
The Position:
Catholic Charities of the Archdiocese of St. Louis, service line St. Patrick Center, provides clientcentered support to individuals seeking stability and longterm success. In this role, you will assess client eligibility, identify strengths and needs, and help clients secure stable housing. You will conduct inhome visits, assist with essential needs such as transportation and food, and guide clients in developing monthly budgets that promote sustainability. Collaboration is key, and you will work closely with the ICM team, SSVF Peer Support, and other community partners to ensure clients receive holistic, coordinated care. This position maintains timely and accurate documentation in CaseWorthy, manages organized client files, and contributes to meeting program goals outlined in the SSVF grant. You will also provide referrals to resources addressing mental health, substance use, legal needs, VA benefits, and additional SPC programs when appropriate. This role is ideal for a motivated, organized professional who is passionate about making a meaningful difference in the lives of others while supporting the overall success of the program. Other duties may be assigned as needed. Our core working hours are Monday through Friday, 8:00 am to 4:30pm.
