Description
GENERAL RESPONSIBILITIES:
- Operate as a member of the Community Services team.
- Conduct comprehensive needs assessments and coordinate services for clients.
- Provide rapid re-housing counseling, consultation, training, crisis intervention, follow-up, and case management services.
- Manage caseload of households, providing ongoing support services including case management, goal planning, resource coordination, and advocacy to promote housing stability and self-sufficiency.
- Help newly homeless persons and those at risk identify and implement strategies to prevent shelter entry.
- Develop creative housing solutions such as family mediation, rental agreements, and conversations with landlords.
- Coordinate services such as transportation for clients with alternative housing locations.
- Engage client family members and friends to explore housing alternatives.
- Maintain excellent communication with shelters, homeless service providers, partner agencies, and first responders.
- Establish care or treatment plans to enable clients to live independently.
- Cultivate relationships with landlords, assist with tenant and landlord issues, and ensure housing and maintenance issues are addressed.
- Other duties as assigned within the guidelines of this position.

Requirements
- Bachelor's degree in social work, psychology, or related human services field required; high school diploma with four years of related experience may substitute.
- Minimum of 1 year of case management experience required.
- Experience with program-specific populations preferred.
- Experience working with low-income and homeless persons.
- Ability to work independently and respond quickly in times of crisis.
- Ability to work effectively with diverse ethnic, racial, and social backgrounds.
- Proficiency in Microsoft Office applications.
- Strong verbal and written communication skills.
- Passage of background clearance, valid driver's license, reliable transportation, and proof of liability insurance.
