Case Manager
The Clinical Case Manager is part of a large continuum of BH services including prevention and works closely with clients and natural supports, treatment teams, and within systems to provide support, linkage to services, and high engagement with treatment interventions. Effective rapport building, outreach and engagement are needed to improve functioning and to establish wellness.
Responsibilities:
- Ensure that the activities of this position and relevant programs are consistent with the mission, vision, and values of Catholic Charities, Diocese of Cleveland.
- Work closely within treatment teams, CCDCFS, Criminal Justice officials and courts, to provide support, community linkages
- Provide clients direct service, either in the office or in the community.
- Service coordination and needs assessment for basic needs.
- Provide advocacy on behalf of clients.

- Maintain documentation compliance within the electronic health record daily.
- Attend clinical and administrative staff meetings, multi discipline team meeting as required.
- Review treatment plans and progress of cases with innovative and relevant interventions to help clients meet their goals.
Requirements:
- Combination of education and experience normally represented by a Bachelor's Degree and one to two years related professional experience.
- application and training plan for a prevention specialist credential if does not have a LSW
- Must have competent oral, written and interpersonal communication skills.
- Must be well organized, detail oriented and have the ability to maintain confidentiality.
- Ability to work with diverse client population, community organizations as well as staff at all levels.
- Must be able to form good working and therapeutic relationships.
- Final applicant is required to be fingerprinted to complete background check.
