What are the responsibilities and job description for the Case Manager position at Catholic Charities ccw?
We are seeking a Case Manager to join our team! The Family Shelter provides temporary emergency shelter when circumstances resulting in homelessness cannot be avoided. We support families with a combination of case management and direct service, provided in an environment that affirms the dignity of the individuals being served.
This position provides intake, develops needs assessments, develops and implement service plans for families and individuals that are at risk for hunger and homelessness.
Education/Experience:
- Bachelor’s Degree in social work, human services or a related field with supervised experience in homelessness, housing search and/or human services; OR a high school diploma, or its educational equivalent, with at least three years of supervised experience in the previously stated fields.
- Ability to read, write, and speak English as it relates to the job; Spanish fluency is a plus.
Licenses/Certifications:
- Valid Driver’s License
PHYSICAL/MENTAL REQUIREMENTS
- Walking and climbing are required for entry and mobility throughout all offices of Catholic Charities Worcester County.
- Normal range of vision and hearing, with or without correction.
- Must demonstrate good judgement in decision-making and determination of information that should be communicated to staff, other supervisors, program managers, etc.
