What are the responsibilities and job description for the LEAD Case Manager position at Catawba Valley Healthcare?
Catawba Valley Healthcare seeks a full time case manager with at least two year’s experience to work as part of a community team based out of Hickory NC. Preference for individuals with substance use and law enforcement experience. This position will provide case management services to the individuals who meet criteria and choose to participate in the Law Enforcement Assisted Diversion (LEAD)program. This position is based in Catawba, NC. This position will complete initial LEAD screenings with individuals and develop a treatment plan based off of needs identified in the screening. The case manager will link the individual to resources which will address the needs. The case manager will monitor services the individual is involved in and will report back to the appropriate law enforcement agency when the individual does not follow through with requirements or successfully completes the requirements for LEAD.
Bachelor's Degree and meets qualifications for a NC qualifies professional.
