Now Hiring: Care Manager - Adult Home (75 Beds)

Pay: $25-$28 per hour (based on experience)

We are seeking a compassionate, organized, and dedicated Care Manager to join our team at a 75-bed adult home. The ideal candidate will have strong communication skills, leadership ability, and a passion for helping residents maintain the highest possible quality of life.

Responsibilities include:

• Coordinate and oversee resident care plans and services
• Communicate with residents, families, healthcare providers, and staff
• Monitor residents’ needs and ensure proper follow-up
• Assist with admissions, assessments, and ongoing resident support
• Maintain accurate documentation and records
• Coordinate medical appointments, services, and outside providers
• Help ensure compliance with facility policies and regulatory requirements
• Work closely with administration and department leaders to support daily operations
  
  
  

Qualifications:

• Experience working with seniors, adult homes, assisted living, healthcare, or case management preferred
• Strong organizational and problem-solving skills
• Excellent communication and interpersonal abilities
• Ability to multitask in a busy environment
• Caring, patient, and professional attitude
• Computer and documentation skills required
