Duties
- Conduct comprehensive assessments of patients’ physical, cognitive, and emotional functioning using evidence-based practices.
- Develop individualized care plans that incorporate therapeutic interventions tailored to each patient’s needs.
- Assist with activities of daily living (ADLs) such as dressing, grooming, feeding, and mobility to promote independence.
- Collaborate with multidisciplinary teams—including physicians, speech therapists, and social workers—to coordinate effective care strategies.
- Manage medical documentation accurately within electronic health records (EHR) systems while adhering to medical terminology standards.
- Engage in case management activities to monitor progress and adjust treatment plans as needed.
Requirements
- Valid licensure as an Occupational Therapist in the relevant jurisdiction with current certification.
- Strong knowledge of physiology and anatomy to inform effective treatment approaches.
- Familiarity with medical terminology and proficiency in managing electronic health records (EHR).
- Demonstrated ability to develop comprehensive care plans aligned with evidence-based practices.
- Excellent communication skills for collaborating with patients, families, educators, and healthcare teams.
- Ability to work effectively in patient's home.
