POSITION OBJECTIVE
Working independently, the Data Clerk 4 will perform clerical tasks to support the operations group by inputting visit data and prepping records for upcoming visits. Provide input, planning, coordination, and maintenance for the Patient Care and Research Database (PCRD) working group for the Center for AIDS Research at the Case Western University/University Hospitals of Cleveland.
Essential Functions
Department: Daily contact with Patient Care and Research Database team members. Regular contact with physicians, other healthcare providers and ancillary personnel in the Special Immunology Unit. Regular contact with investigators, research nurses and laboratory personnel.
University: Some contact with Case Western Reserve University administrative personnel, payroll and HUB personnel to ensure the administrative, budgetary and compliance needs are met.
External: May have contact with outside consultants, other institutions and representatives needed to fulfill the requirements of the multicenter projects.
Students: Little or no contact with students.
SUPERVISORY RESPONSIBILITY
This position has no direct supervision of staff employees. However, this position does lead junior level clerks in day-to-day activities.
Qualifications
Education/Experience: High School education and 4 to 5 years of related experience OR an Associate's degree and 0 to 1 year of experience required.
Required Skills
General office environment. The employee will perform repetitive motion using computer mouse and keyboard to type. Limited manual dexterity required. Work requires negligible physical exertion. Little or no exposure to hazards.
This position is eligible for hybrid work arrangement up to two remote days per week at the discretion of the department. New employees may begin a hybrid schedule upon approval from the supervisor, successful completion of an orientation period and signing the remote work checklist certification form.

Essential Functions
- Enter clinical data derived from provider-patient encounters within the Special Immunology Unit (SIU) including data from the University Hospitals electronic medical record and research participants in studies conducted at the clinical research site. (20%)
- Conduct regular extraction of relevant data from electronic medical record and printed medical records for all patients followed in the Special Immunology Unit and research participants whose records are to be maintained through the Patient Care and Research Database. (20%)
- Devise procedures and make recommendations for the processing involved in maintaining complex record-keeping and/or filing systems such as the Patient Care and Research Database. (10%)
- Work with department managers in identifying needs as they relate to attaining reports and information needed by investigators/clinicians. (6%)
- Facilitate the flow of information to and from researchers who request data from the Patient Care and Research Database and coordinate with the repository to obtain any needed samples. (10%)
- Maintain ongoing contacts with investigators/clinicians to solve problems and resolve issues (i.e., lab imports, screens, etc.). May answer telephone and receive visitors. (6%)
- Assist with compliance, design and enforcement of the policies related to HIPPA compliance, patient confidentiality, data integrity and patient consent. Assist with periodic reviews of data quality in the Patient Care and Research Database, including all members of the database team and additional ad-hoc participants as needed. (15%)
- May be responsible for maintaining inventory of office and clinical supplies, reordering and stocking supply closets. (5%)
- May lead junior level clerks in day-to-day activities. (2%)
- Pick up and deliver mail or other correspondence from the Special Immunology Unit. Perform copying and sorting duties as related to the print outs of data for the medical clinics daily. (5%)
