ABOUT CASCADE:

Cascade Community Healthcare (CCH) has been providing treatment services to Lewis County and surrounding areas for more than 50 years. Our mission is to provide our clients with compassionate and encouraging counseling leading to a renewed sense of hope and the ability to make positive changes and live more satisfying lives. Cascade currently has six treatment locations in Lewis County. The Cascade Evaluation and Treatment Center provides intensive mental health and psychiatric services for adults 18 and older. This 22-bed facility has 16 beds on the evaluation and treatment unit, and 6 beds on our crisis stabilization unit.

 

We are looking for a Clinical Support Coordinator to join our team. The Clinical Support Coordinator works closely with an interdisciplinary treatment team to coordinate care and discharge planning for individuals, their families, local providers, and other referral sources.

 

SHIFTS AVAILABLE:

  • Day Shift, Sunday - Tuesday: 10:00am - 10:30pm
  • Day Shift, Wednesday - Friday: 10:00am - 10:30pm
  • Day Shift, Friday - Sunday: 10:00am - 10:30pm
  • NOC Shift, Sunday - Tuesday: 10:00pm - 10:30am
  • NOC Shift, Wednesday - Friday: 10:00pm - 10:30am 
  • NOC Shift, Friday - Sunday: 10:00pm - 10:30am


This position is eligible for $2/hour shift differential for eligible hours on Swing an NOC shifts and hours worked on Saturdays, Sundays, and holidays.

 

PAY SCALE:

$19.72 - $24.03, DOE.

 

WHAT YOU NEED TO GET THE JOB DONE:

  • Washington State Registered Agency Affiliated Counselor (RAAC) credential required, or eligible to obtain.
  • Must be able to pass WA State Patrol Background check at hire and annually thereafter. May require National background check if haven’t lived in Washington State consecutively the last three years.
  • Ability to communicate effectively both verbally and in writing.
  • Ability to work dynamically as part of a multi-disciplinary team.
  • Ability to develop and maintain effective working relationships with co-workers, public and private sector organizations, community groups, and the public.
  • Must maintain current phone and mailing address with the Human Resources Department.



WHAT YOU'LL DO:

  • Educates and informs individuals of consents for treatment and authorizations for use and disclosures.
  • Participates regularly in interdisciplinary treatment team meetings.
  • Collaborates with the interdisciplinary team and patient on providing appropriate resources and coordinating external appointments for patient.  Work with the patient on what natural resources that they have by conducting a discharge assessment. Actively coordinate care and provide referrals for individuals to ensure services are coordinated with both facility staff and external providers.
  • Identify and address gaps in service needs for participating parties in service delivery to the individual; and make appropriate recommendations to meet individuals needs upon discharge.
  • Collaborate with interdisciplinary treatment team to engage, monitor and communicate with the individual ensuring that safety is maintained while meeting the individuals' needs.
  • Maintain the clinic record, including documentation of activities performed as part of the service delivery process. Obtain signatures from individuals regarding treatment, as necessary. Document all services and individuals' activities in the medical record regarding brief interventions, coordination of care, discharge planning, treatment planning, and any other pertinent information.

 

PHYSICAL DEMANDS

  • Occasional to considerable standing or walking, varies based on acuity of unit.
  • Protective holds, movements and carries.
  • Blocking and redirecting patient movements.
  • Physical interventions in response to aggressive and self-injurious behavior.
  • Ability to move quickly and confidently to intervene in situations when protective holds and/or movements are deemed necessary.
  • Ability to pursue a client in the event they try to leave the secure unit.
  • Successful completion of MOAB training (Management of Aggressive Behavior).
    • Including but not limited to pivoting, knee squats, shuffling front and backwards, assisting a person to the floor, stabilizing people in standing, seated, lying and supine positions.
  • Lifts, positions, pushes, and/or transfers patients.
  • Occasional reaching, stooping, bending, kneeling, and crouching.
  • While performing the duties of this job, the employee is regularly required to see, talk, and hear.
