Community Youth Services (CYS) is seeking a passionate and dedicated Transitional Age Youth (TAY) Clinical Care Coordinator to join our behavioral health team and make a meaningful impact in the lives of young people. This role provides intensive, home- and community-based Clinical Care Coordination to youth navigating complex systems, helping them build the tools and confidence to lead their own treatment journey.

The Clinical Care Coordinator delivers individualized, flexible services to youth, including evening and weekend appointments as needed. They are trained in WISe (Wraparound with Intensive Services), Positive Youth Development, MAP WRAP, safety planning, and CANS assessments. Working as part of a compassionate, collaborative team, they help bridge communication gaps, build natural supports, promote healthy boundaries, and foster positive change. Their approach is youth-centered, culturally responsive, and focused on inspiring hope and achieving meaningful outcomes.



Key Responsibilities:

  • Deliver youth-driven care coordination using Wraparound principles and MAP WRAP tools.
  • Serve as the central communication hub for the WISe team.
  • Maintain a caseload of 10–15 youth participants.
  • Conduct CANS assessments and develop individualized plans of care.
  • Facilitate culturally responsive engagement and rapport-building with youth and families.
  • Coordinate and lead Wraparound Team Meetings, ensuring inclusive, outcome-focused collaboration.
  • Track service hours and document care using the agency’s Electronic Health Record (EHR) system.
  • Promote team culture that is emotionally safe, culturally humble, and strength-based.
  • Collaborate with Youth and Family Peer Specialists to identify supports and resources.
  • Align all services with WISe’s 10 guiding principles, including youth voice, natural supports, and unconditional care.
  • Support youth through all phases of WISe and assist in developing sustainability plans for long-term success.



Qualifications:


Required Education & Credentials

  • Bachelor’s degree in Social Work, Psychology, Counseling, or related field
    or Associate’s degree with 2 years of relevant experience
  • Must hold a Washington State Agency Affiliated Counselor credential

Professional Experience

  • Experience coordinating care across multiple systems: mental health, juvenile justice, schools, DCYF, DSHS, DVR, and community supports
  • Familiarity with legal systems affecting youth and families (e.g., dependency orders, juvenile court, DCYF procedures)

Cultural & Clinical Competency

  • Knowledge of cultural issues impacting marginalized youth and alignment with Positive Youth Development and Wraparound principles
  • Ability to maintain a non-judgmental, culturally humble approach

Skills & Abilities

  • Strong initiative, organization, and time management to meet deadlines
  • Skilled in facilitating meetings, presenting, coordinating teams, and building relationships across diverse populations
