Homedica is seeking a compassionate and skilled PRN Nurse Practitioner to deliver comprehensive primary care services across a variety of settings—including in-home visits, assisted living and nursing facilities, post-acute care hospitals, and via telehealth.

Responsibilities:
- Provide primary medical care services in home, facility, and telehealth settings
- Conduct physical exams, wellness visits, and manage chronic and acute conditions
- Order and interpret diagnostic tests and labs
- Prescribe and manage medications and individualized care plans
- Coordinate care with interdisciplinary teams and community partners
- Promote preventive care, patient education, and continuity of care
- Participate in after-hours or on-call rotation as needed
Qualifications:
- Active Nurse Practitioner license in Louisiana
- DEA license required
- Experience in home health, hospice, or facility-based care required
- Strong communication, decision-making, and clinical skills
- Comfortable working independently across diverse care environments
