Carolina Caring is seeking a Referral Navigator to join our compassionate, mission-driven team! In this rewarding role, you'll serve as a vital connection between patients, families, referral partners, and our clinical teams, helping ensure timely access to Hospice, Palliative Medicine, House Calls, and Cardinal Kids services. As the first point of contact for many patients, you'll coordinate referrals, obtain physician orders, verify insurance benefits, schedule admissions and consultations, and ensure accurate documentation in our electronic medical record. Schedule:
Monday–Friday, 9:30 a.m.–6 p.m.
Rotating every 9th Saturday, 8:30 a.m.–5:00 p.m.

Full-Time (40 hours per week)
