What are the responsibilities and job description for the Foster Care Coordinator position at Carolina Adoption Services?
Basic Function:
To provide professional social work / case management services for foster parent applicants, licensed foster families, and children within care through evaluation, case management, strength-based support, and referrals.
Duties and Responsibilities:
- Work involves evaluating the children within care and his/her ability to cope; develop a social history, psychosocial assessment, service plan and/or treatment plan and follow up. Services may include basic counseling, support, and reinforcement in areas including, but not limited to, mental health, grief, vocational, and behavioral issues.
- Conducts recruitment, evaluation, Mutual Home Assessment, licensing and relicensing, and training of foster parents and other care providers.
- Responsible for referring clients and families to a variety of services to meet financial, psychological, emergency care, emotional, or other needs, as applicable.
- Responsible for completing independently performed assessments / evaluations to identify needs and establish an appropriate service/treatment plan or assessment and may provide basic supportive/directive counseling to clients and families in addressing needs.
- Conducts a minimum of one face-to-face contact with foster families, and the child/children in care, per month.
- Ensures that required documentation from the foster parents is completed and retrieved on a minimum of a monthly basis.
- Documents direct client interactions, service development, assessments/evaluations, and referrals in accordance with agency requirements.
- Provides 24 hour on call support to foster parents as assigned by supervisor.
- Provides services performed under supervisory guidance and participates in bi-weekly supervision.
- Maintains other duties assigned
Specific Knowledge, skills and abilities required:
- Master’s degree in social work from an appropriately accredited institution; OR Bachelor’s degree in social work from an appropriately accredited institution and one year of directly related experience; OR Master’s degree in a human services field from an appropriately accredited institution and one year of directly related experience; OR Bachelor’s degree in a human services field from an appropriately accredited institution and two years of directly related experience; OR an equivalent combination of education and experience.
- A valid Driver’s License is required
- Work requires an understanding of social work practices, theories, principles, techniques, and their application to individual casework.
- Must be able to build rapport with clients and assess psychosocial, socioeconomic, psychological, and behavioral needs. Must maintain a positive working relationship with designated caseload.
- Must have an understanding of the availability and interactions of a variety of community services, resources, agencies, and service providers.
- Must have basic knowledge of medical terminology and disease processes and/or special needs / developmental programs and behavior management techniques.
- Required to utilize knowledge of a range of services and of supportive level counseling and other counseling techniques to provide services to clients.
- Must adhere to agency guidelines/policies, state guidelines/standards per regulatory requirements, and ethical standards of the Social Work profession.
- Must provide excellence in service delivery, practice consistent/detailed documentation, and maintain positive working relationships with supervisor and coworkers.
- Must be able to provide services to the team in a timely, accurate manner with strong customer service focus.
- Requires excellent writing and editing skills with attention to detail.
- Proficiency with Microsoft applications including Word, Excel, and Outlook.
- Must reflect sensitivity and responsiveness to client needs and to cultural differences present in the agency’s service population.
- Ability to express Ideas clearly and concisely and to plan and execute work.
Job Type: Part-time
Pay: $15.00 - $25.00 per hour
Expected hours: 1 – 20 per week
