The Well-Being Department provides primary and acute healthcare, physical therapy, occupational therapy, speech and language pathology, social work services, pastoral counseling, life enrichment and fitness opportunities. The department is seeking a Physical Therapist who will be an integral part of the Carol Woods interdisciplinary healthcare team.
PRIMARY FUNCTION:
Plan, and deliver physical therapy services in accordance with current applicable federal, state and local standards, guidelines and regulations to ensure the highest degree of quality resident care is consistently maintained.

RESPONSIBILITIES AND DUTIES:
- Plan, develop, organize, implement, evaluate and manage physical therapy services.
- Maintain physical therapy standards.
- Assist in developing, implementing and coordinating policies and procedures, resident care plans, physical therapy procedure manuals.
- Develop, supervise and assist in group exercise and fitness classes, including maintaining appropriate documentation requirements.
- Maintain an excellent working relationship with the medical profession and other health care facilities and organizations.
- Review request for physical therapy and physicians’ orders. Interview resident to determine type of treatment and schedule therapy.
- Participate in discharge planning, development and implementation of resident care plan, resident assessments and selection of assistive devices as necessary.
- Conduct functional assessments of residents for purposes of determining level of service needs or to recommend interventions to delay decline.
- Participate in interdisciplinary team meetings and other work teams, as assigned, which may deal with the functional abilities/challenges of residents.
- Perform administrative requirements
- Participate and assist in departmental studies and projects as assigned.
- Provide physical therapy in resident’s room in HC when appropriate.
- Evaluate and implement recommendations from the standing committees of Carol Woods as requested.
- Provide rehab-related in-services to CW staff as requested.
- Work with the Manager of Business Services and the Director of Clinic Services in identifying equipment and supply needs for the Rehab. Dept.
- Assure the Rehab. Dept. is cleanly and safely maintained for both staff and residents including all equipment and supplies maintained in the Rehab Department.
- All other duties as assigned.
REQUIREMENTS:
Education:
- Bachelor of Science degree in Physical Therapy with a current State of North Carolina license as a physical therapist.
Experience:
- Preferred two years as a licensed physical therapist in a healthcare facility, preferably in long term care.
