FULL-TIME DAY SHIFT (7A-7P) AND FULL-TIME NIGHT SHIFT (7P-7A) NEEDED!

Join our dedicated healthcare team as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) specializing in Long Term Care. In this vital role, you will provide compassionate, comprehensive nursing care to residents in a long-term care setting, ensuring their comfort, safety, and well-being. You will be instrumental in delivering high-quality patient care through assessments, medication administration, and collaboration with interdisciplinary teams. Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and health histories to develop personalized care plans.
• Administer medications and treatments accurately, including injections, IV infusions, tube feedings, and wound suturing, adhering strictly to aseptic techniques and safety protocols.
• Monitor residents’ conditions closely using telemetry and other monitoring systems; respond promptly to acute changes or emergencies such as trauma or critical illness.
• Document all patient interactions and treatments precisely within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations.
• Collaborate with physicians and interdisciplinary teams on diagnostic evaluations, medication adjustments, discharge planning, and complex case management involving behavioral health or memory care needs.
• Provide specialized care for diverse populations including geriatrics, individuals with developmental disabilities, hospice patients, and those requiring trauma or infectious disease management.
• Support residents’ activities of daily living (ADLs), including catheter care, ostomy management, and tube feeding while maintaining dignity and respect.

Qualifications

• Current licensure as an RN or LPN in good standing; additional certifications such as Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS), or Pediatric Advanced Life Support (PALS) are preferred.
• Proven experience working in long-term care facilities or nursing homes; experience with memory care or behavioral health is highly desirable.
• Familiarity with electronic health records (EHR) systems such as Epic, Cerner, Athenahealth or eClinicalWorks; proficiency in medical documentation and ICD coding (ICD-10/ICD-9).
• Strong knowledge of medical terminology, physiology, aseptic techniques, infection control practices, and medication administration protocols.
• Ability to perform complex patient assessments including vital signs monitoring and laboratory specimen collection/processing.
• Experience working with individuals with disabilities or chronic conditions requiring specialized interventions like dialysis or ventilator management is advantageous.
• Excellent communication skills combined with critical thinking abilities to triage urgent situations effectively.
