In this role, you will provide on-site support to multiple centers, assist during transitions, cover leaves of absence, support survey readiness, and ensure regulatory compliance across facilities.
Key Responsibilities:
- Provide Social Services coverage and support to assigned skilled nursing facilities
- Complete psychosocial assessments, care plans, MDS sections, and discharge planning
- Support residents and families with counseling, resources, and care coordination
- Assist facilities with survey preparation and regulatory compliance
- Ensure adherence to federal, state, and company policies
- Collaborate with interdisciplinary teams to promote resident-centered care
- Provide mentoring and guidance to facility-based Social Services staff as needed
- Bachelor’s Degree in Social Work or related field required (MSW preferred)
- Current Virginia licensure (if applicable)
- Minimum 2 years of experience in skilled nursing or long-term care
- Strong knowledge of CMS regulations and MDS process
- Ability to travel within the Richmond region
- Excellent communication, documentation, and organizational skills
