Office/Care Coordinator

The Office/Care Coordinator supports home care and senior living services for older adults and adults with disabilities. Reporting to the Operations Manager, this role coordinates care and client schedules, manages office operations and records, and serves as a liaison among clients, caregivers, and the organization. The position is primarily community-based and office-based, with occasional client visits.

Responsibilities

• Coordinate home care and senior living services for clients.
• Manage client scheduling and organizational calendars.
• Coordinate intake activities and client communications.
• Serve as a liaison between clients, caregivers, and internal teams.
• Maintain accurate client records and documentation.
• Provide office administration support.
• Track compliance requirements and related activities.
• Conduct occasional client visits as needed.

Preferred Qualifications

• 3 years of experience in care coordination.
• High school diploma or equivalent.
• Knowledge of HIPAA compliance.
• Experience with scheduling systems and documentation management.
• Strong communication and problem-solving skills.
• Bilingual in English and Spanish (preferred)
