Job Overview
We are seeking dedicated and experienced Licensed Practical Nurses (LPN) and Registered Nurses (RN) to join our healthcare team for 7-3, 3-11, and 11-7 shifts. This position offers an opportunity to provide high-quality patient care across various settings, including acute care, long-term care, assisted living, and specialty units. The ideal candidates will possess strong clinical skills, comprehensive knowledge of medical procedures, and a compassionate approach to patient-centered care. Experience with EMR/EHR systems such as PointClickCare is highly desirable. This role is vital in delivering safe, effective, and personalized nursing services to diverse patient populations.
Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and medical history documentation.
- Administer medications and injections accurately following physician orders while adhering to HIPAA and infection control protocols.
- Manage IV infusions, catheterizations, tube feeding, as required by patient needs.
- Document all patient interactions and procedures thoroughly within electronic health records (EHR/Epic).
- Provide specialized care including Alzheimer’s/dementia care, memory care, hospice support, pain management, and acute or chronic illness management.
- Support discharge planning, case management activities, and coordinate with interdisciplinary teams for optimal patient outcomes.
- Educate patients and families on health coaching topics including nutrition, medication administration, post-acute care instructions, and disease management.
Experience
- Proven experience in Long-term care settings provided.
- Demonstrated proficiency with EMR/EHR systems.
- Skills in advanced nursing procedures such as phlebotomy/venipuncture, sterile processing techniques, dialysis support, or dermal fillers are advantageous.
- Knowledge CPT coding for procedures; familiarity with discharge planning and post-acute care processes is beneficial.
- Strong understanding of anatomy/physiology principles along with competency in medical terminology and diagnostic evaluation methods.
- Experience working with diverse populations including seniors requiring geriatrics or memory care services; This role requires licensed nurses committed to delivering compassionate patient care while maintaining the highest standards of safety and professionalism across all shifts—7am to 3pm, 3pm to 11pm, or 11pm to 7am—ensuring continuity of care around the clock.
Pay: $45.00 - $60.00 per hour
