Your Role as an MDS Nurse:
- Conduct initial and ongoing MDS assessments in accordance with federal, state, and facility regulations
- Accurately complete all sections of the MDS, including RAI and care planning documentation
- Collaborate with the interdisciplinary team to ensure accurate documentation of resident needs and goals
- Provide education and guidance to staff on MDS requirements and regulatory standards
- Ensure compliance with all federal and state regulations related to long-term care
- Participate in quality improvement initiatives and regulatory survey processes
- Maintain accurate, up-to-date records of MDS assessments and care plans
- Coordinate timely transmission of MDS data to appropriate agencies
- Participate in interdisciplinary care plan meetings and provide clinical input
- Monitor and update care plans based on changes in resident condition
- Maintain case-mix index at target levels
- Communicate effectively with physicians, staff, residents, families, and vendors
- Follow all facility policies, including infection control, safety, and HIPAA regulations
- Maintain a calm, supportive, and professional environment
- Attend required trainings, in-services, and meetings
- Graduate of an accredited School of Nursing
- Current Ohio LPN or RN license
- Current CPR certification
- Minimum of 1 year of experience as an MDS Nurse required
- 3 years of experience in long-term care or skilled nursing preferred
- Strong knowledge of federal and state long-term care regulations
- Excellent attention to detail and accuracy
- Strong organizational and time-management skills
- Effective communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in Microsoft Office and electronic medical records (PointClickCare preferred)
- Physically able to meet job requirements, including lifting and assisting residents as needed

Salary : $35 - $40
