Your Role as a Case Manager:
- Coordinate resident care from admission through discharge planning
- Complete discharge planning assessments within 48 hours of admission
- Develop and manage individualized discharge plans of care
- Serve as a liaison between interdisciplinary teams to ensure coordinated care
- Educate residents and families on diagnoses, medications, and use of medical equipment using the Teach Back Method
- Assist with Advance Directives and ensure appropriate documentation is obtained
- Coordinate insurance updates and manage appeals for denials
- Schedule follow-up appointments and arrange post-discharge services, including DME, medications, and home health
- Ensure all discharge documentation and instructions are complete and communicated clearly
- Conduct post-discharge follow-ups at 1 week and 30 days
- Communicate discharge plans and updates with leadership and care teams
- Supervise unit staff and support adherence to care standards and resident rights
- Assist with activities of daily living as needed
- Maintain a safe, clean, and supportive environment for residents and staff
- Graduate of an accredited School of Nursing
- Current LPN or RN license in the State of Ohio
- Current CPR certification
- 5 years of experience in healthcare or long-term care preferred
- Strong care coordination, organizational, and problem-solving skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively with a team
- Compassionate, patient-focused approach to care
- Ability to manage multiple priorities in a fast-paced environment
