Hospice Registered Nurse Case Manager

We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our hospice care team. In this role, the RN Case Manager will coordinate and deliver comprehensive hospice care by collaborating closely with the interdisciplinary team (IDG), patients, and their families to ensure all physical, emotional, and spiritual needs are met.

Key Responsibilities:

• Utilize the nursing process to provide skilled and compassionate nursing care to hospice patients.
• Collaborate with the attending physician and Hospice Medical Director regarding any significant changes in the patient’s condition that may require updates to physician orders or the plan of care.
• Coordinate all aspects of patient care, including services provided by other members of the IDG and contracted providers, as needed.
• Accurately document all services rendered, changes in the patient’s condition, and modifications to the nursing care plan.
• Conduct and document supervisory visits with the Hospice Aide; evaluate the quality of care and patient progress.
• Prepare and update the Hospice Aide Plan of Care based on patient needs.
• Participate in interdisciplinary team meetings (IDG) and staff development sessions to support continuous quality improvement and collaboration.
• Ensure timely and accurate chart closure for discharged patients.

Qualifications:

• Current Registered Nurse (RN) license in good standing.
• Previous hospice or home health experience preferred.
• Strong clinical assessment, organizational, and communication skills.
• Ability to work independently and as part of a team.
• Compassionate and empathetic approach to end-of-life care.
