Hospice Registered Nurse Case Manager
Caring Hearts Hospice Inc. is looking for a Registered Nurse Case Manager to join our hospice team. This is a role for someone who genuinely enjoys the clinical side of nursing but also wants to be fully present with families during one of the hardest chapters of their lives.
As the RN Case Manager, you'll share and manage a caseload of hospice patients within a 60 mile radius of our office. You'll work directly with physicians, our Hospice Medical Director, and the rest of the interdisciplinary team to keep each patient's plan of care current and appropriate as their condition changes. A lot of the job comes down to good clinical judgment, clear communication, and staying organized across multiple patients and families at once.
Day to day, you can expect to:
- Provide skilled nursing care using sound clinical judgment and the nursing process
- Talk with attending physicians and our Medical Director about changes in a patient's condition and what that means for their care plan
- Coordinate care across the whole team, including services from IDG members and outside providers
- Conduct and document supervisory visits with Hospice Aides, evaluating the quality of care they provide and the patient's progress
- Prepare and update each Hospice Aide's Plan of Care based on the patient's needs
- Keep accurate, timely documentation, from visit notes to chart closures when a patient is discharged
- Take part in IDG meetings and ongoing staff development
You'll have a full team supporting you along the way, chaplains, social workers, aides, and physicians, so you're never figuring things out on your own.
What we're looking for:
- An active RN license in good standing
- Previous hospice or home health experience preferred, though we're open to strong candidates without it
- Solid clinical assessment, organizational, and communication skills
- Ability to work independently while collaborating closely with a team
- A genuinely compassionate, empathetic approach to end of life care
- Possess and maintains current CPR certification if required by state.
- Current driver's license, vehicle insurance, and access to dependable transportation.
Job Type: Full-time
Schedule: Monday - Friday, 8:00 am - 5:00 pm
Compensation: This is a salaried position, paid on a bi-weekly basis
