Immediate opening to join our growing team. SNF Case manager position available for experienced candidate to work on-site at our Caring Heart location. Responsibilities include but are not limited to providing weekly updates to the insurance companies, assist with obtaining exclusions and carve outs, experience with the triple check process, medical terminology, medical classification and assisting the team with quality case outcomes. Must possess excellent customer service skills and the ability to multi-task. Proficiency with Word & Excel is a Plus. The ability to be self-guided and directed would be beneficial.
