POSITION SUMMARY:
The Community Health Worker (CHW) role is a contract position. Responsibilities of this role include facilitating and connecting the vulnerable population to health, social, and mental and behavioral health services; providing culturally relevant health education on chronic disease prevention, physical activity, and nutrition, and advocatingfor the individuals to get health care services and provide resources. Additionally, the CHW is responsible for participating in outreach and enrolling individuals to benefit from our organization's educational activities and community services, providing presentations at local agencies and community events, and tracking individual and group data information in a timely manner.
ESSENTIAL FUNCTIONS & RESPONSIBILITIES:
- Facilitate and connect the vulnerable population to health, social, and mental and behavioral health services.
- Provide culturally appropriate health education on chronic disease prevention, physical activity, and nutrition.
- Advocate for individuals to get health care services and provide resources.
- Provide informal counseling, health screenings, referrals, and appropriate follow up.
- Participate in designing, implementing, and interpreting individual-level assessments, Determinants of Health assessment, a survey of community assets and challenges, and community asset mapping.
- Address Social Determinants of Health.
- Navigate individuals enrolling in programs and receiving benefits for which they qualify or are accessible.
- Outreach and enroll individuals to benefit from our organization's educational activities and community services.
- Collect and track individual and group data and information needed by the programs.
- Submit data on time and as outlined in the scope of work.
- Conduct home visits to provide education, assessment, and social support.
- Educate individuals in disease prevention and self-management of chronic conditions.
- Provide presentations at local agencies and community events.
- Participate in evaluation and research projects (identify and engage community members).
- Participate in all training, workshops, and educational classes as needed to meet the scope of work.
- Complete the scope of work as outlined in special project contracts.
- Be available to work during certain times as outlined in special project contracts.
QUALIFICATIONS:
- High School Diploma or GED
- 1-2 years of work experience
- Authorization to work in US Required
- Flexible schedule
- Ability to document and report on data.
- Strong critical thinking skills, adaptability, and empathy.
- Strong interpersonal, oral, and written communication skills.
