RN Case Manager

• This is a non-clinical, case management position—no hands-on care required.
• You’ll be responsible for start-of-care visits, home-based assessments, and ongoing client oversight. The role is based out of our Bethlehem office and involves light local travel—typically 3 home visits per day for quick head-to-toe assessments.

Qualifications:

• Registered Nurse License in the state of Pennsylvania
• At least one year of nursing experience required.preferrably in a home health experience
