About This Role
We are seeking dedicated, licensed Physical Therapists to provide home health services in Boca Raton, Delray, Boynton Beach and West Palm Beach. In this role, you will deliver one-on-one care, assess patients’ therapeutic and functional status, develop and direct individualized treatment plans, and educate patients and caregivers on safe, effective care.
You will collaborate closely with physicians, nurses, and other care team members to ensure coordinated, high-quality services, while enjoying the flexibility to manage your schedule and the satisfaction of seeing the real impact of your care.
Key Responsibilities
- Assess and evaluate therapeutic/rehabilitative/functional status and participate in the development of the total plan of care. Evaluate the home environment.
- Assesses for muscle strength, mobility, gait, ROM—potential for rehab.
- Direct physical therapy treatment.
- Instruct patients and families/caregivers in the use and care of therapeutic appliances.
- Determine priority needs for physical therapy
- Report to physicians on patients’ reaction to treatment or changes in condition.
- May train patients in the use of prosthetic devices.
- Identify patient and family/caregiver needs for other home health services and referrals as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals.
- Participate in discharge planning for patients.
- Prepare and submit a clinical progress summary based on the attainment of goals directed by Organization policy.
- Provide physical therapy consultation to home families/caregivers when indicated.
- Provide service education programs for nursing organization personnel as needed.
- Participate in the peer consultation process

Qualifications
- Current Physical Therapist license in Florida
- Graduate of an accredited Physical Therapy program
- Strong clinical assessment, communication, and documentation skills
- Valid driver’s license and reliable transportation
- Current CPR/BLS certification
- Minimum 1 year of clinical experience; home health experience

Salary : $73,826 - $109,576
