- Licensed Practical Nurse?( LPN?) provides direct patient care as defined in?the State?Nurse Practice Act.?
- LPN implements?a plan of care?initiated?by the?clinician.?
- Provides?accurate?and?timely?documentation consistent with the plan of care.?
- Licensed Practical Nurse?( LPN?) assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.?
- LPN?participates?in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, and Dietician) to the clinician and/or Clinical Supervisor.?
- LPN uses equipment and supplies effectively and efficiently.?
- Licensed Practical Nurse?( LPN?)?participates?in personal and professional growth and development.?
- Performs other duties as assigned by the Registered nurse.?
- Currently licensed?as a?Licensed?Practical?Nurse in Pennsylvania?
- Valid?CPR Certificate.?
- At least two years experience working with pediatric patients.?
- Compliant?with?accepted professional standards and?practices.?
- Demonstrates good verbal and written communication, and?organizational?skills.?
