- Assess patients’ psychosocial status in relation to illness, environment, and support systems
- Communicate findings to the Registered Nurse and interdisciplinary care team
- Develop and implement social service intervention plans
- Maintain accurate and timely clinical documentation
- Provide patients and families with guidance and referrals to community resources
- Serve as a liaison between patients, families, and community agencies
- Assist physicians and care team members in understanding social and emotional factors affecting health
- Participate in case conferences, care planning, and discharge planning
- Build and maintain relationships with community resources and agencies
- Perform additional duties as delegated by the Clinical Director/Supervisor
- Master of Social Work from a program accredited by the Council on Social Work Education
- Licensed Clinical Social Worker (LCSW) required
- Minimum of one (1) year of healthcare experience
- Home health experience preferred
- Strong verbal and written communication skills
- Excellent organizational skills
- Current CPR certification
