- Evaluate patients’ functional abilities including strength, endurance, coordination, communication skills, and self-care capacity
- Assess home environments for safety risks and recommend modifications or adaptive equipment
- Develop and implement individualized treatment plans with measurable functional goals
- Communicate plans of care with physicians and interdisciplinary healthcare team members
- Provide therapeutic interventions to restore daily living skills affected by illness or injury
- Fabricate splints and train patients in the use of adaptive, prosthetic, and orthotic devices
- Perform work capacity evaluations when appropriate
- Maintain timely, accurate clinical documentation
- Educate patients, families, and caregivers on treatment plans and progress
- Participate in case conferences, care coordination, and discharge planning
- Current state licensure as an Occupational Therapist in PA
- Registered with the National Registration Examination of the American Occupational Therapy Association
- Minimum of one (1) year of clinical experience preferred
- Strong clinical documentation and organizational skills
- Excellent verbal and written communication abilities
- Current CPR Certification
- Ability to work independently in a home health setting
