Home Health Community Liaison
Careland Home Care is a growing home health agency with a strong team, supportive work environment, and a commitment to providing excellent patient care.
We are looking for an experienced Community Liaison / Business Development professional to join our team and help grow the agency by developing strong referral relationships throughout the community.
Responsibilities:
- Build and maintain relationships with hospitals, skilled nursing facilities, physician offices, assisted living facilities, and other referral sources
- Generate new home health referrals and help grow agency census
- Regularly visit and follow up with existing and prospective referral partners
- Educate healthcare professionals about our services
- Identify new referral opportunities and build long-term relationships
- Work closely with our clinical and administrative team to ensure a smooth referral and admission process
- Track referral activity and results
Qualifications:
- Previous home health, home care, hospice, or healthcare business development experience required
- Proven experience developing referral relationships and generating business
- Existing relationships within the local healthcare community are a strong plus
- Strong communication and relationship-building skills
- Goal-oriented and driven by results
- Valid driver's license and reliable transportation

Starting at $70,000 per year, based on experience, plus performance-based bonus opportunities.
