Position Summary

Reporting to the Regional Administrator, the Care Coordinator Lead guides a team of Care Coordinators, providing guidance and support to ensure effective scheduling and management of client care. This role is responsible for handling escalated issues, resolving complex scheduling or client concerns, and ensuring all cases are properly staffed and time sheets are submitted promptly. Serving as a key liaison between clients, aides, the Regional Administrator and others within the Registry, the Care Coordinator Lead maintains up-to-date schedules, fosters clear communication, and facilitates referrals to the Medicare team for skilled care when needed.

Key Responsibilities

  • Manage scheduling for open-client care needs, ensuring all cases within the designated caseload are fully staffed for the following day and weekend
  • Upon client discharge, end the client’s schedule and notify the assigned aide to submit their timesheet
  • Maintain accurate and current schedules, ensuring the correct caregiver, scheduled time, and Admission ID are reflected for each case
  • Follow up on open schedules that have not been confirmed or processed to verify that the aide has worked as scheduled
  • Build and maintain relationships with clients in the designated caseload, ensuring ongoing communication and support
  • Refer patients to the Medicare team for skilled care when appropriate
  • Manage the Emergency Management plan for patients within the assigned caseload
  • Enter daily notes, status changes, and discharge notes in Matrix patient and aide files as needed
  • Inform aides about clients’ needs to ensure care requirements are met
  • Maintain communication with available caregivers, offer open assignments, and update aide availability in Matrix
  • Acknowledge caregiver birthday to maintain engagement and morale
  • Conduct daily and weekly customer service calls to clients to monitor satisfaction and address concerns
  • Assist with handling inbound calls to support client and staff needs
  • Take on-call responsibilities as scheduled
  • Perform weekly follow-ups with clients in facilities to ensure continued quality of care
  • Ride-along with Regional Administrator to meet with clients and family members

Perform other duties and responsibilities as assigned.  

Qualifications

Education & Experience:

  • High School Diploma or GED required, and an associate’s degree preferred in healthcare administration or business
  • Minimum 1-year previous experience in a customer service or healthcare administrative role

Skills & Competencies:

  • Strong organizational and time management skills to effectively handle multiple tasks and priorities
  • Clear verbal and written communication with clients, caregivers, and staff
  • Comfortable using computers and programs like Matrix or similar healthcare software
  • Pays close attention to detail when recording information
  • Can solve problems and think quickly to help clients and staff
  • Keeps information private and acts professionally at all times
  • Friendly and helpful, building good relationships with others
  • Can adjust to schedule changes and is willing to be on-call
  • Works well with others as part of a team

Work Environment:

  • This position requires travel to client homes, participation in community marketing events, and presence in office when not participating in off-site activities. Reliable transportation is required for fulfilling the responsibilities of the role
  • Positivity and a commitment to operational excellence, and a willingness to work cross-functionally is critical to ensure each client experiences “best in home care services” each day
  • This position is designated as an in-person role
