Job Overview

Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility assistance
- Monitor and observe patients for any changes in health status or behavior, documenting findings accurately
- Medication Reminders and manage care plans under supervision, ensuring compliance with HIPAA regulations
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Support patients with mobility using equipment such as Hoyer lifts or resident lifts, ensuring safety at all times
- Perform light cleaning duties to maintain a safe and hygienic environment for residents
- Take vital signs such as blood pressure, pulse, and temperature; report abnormal findings promptly
- Provide companionship and emotional support to residents, fostering a caring environment
- Follow established protocols for patient monitoring and emergency response, including CPR and first aid procedures
- Collaborate with healthcare professionals to develop and implement personalized care plans tailored to each resident’s needs
Skills
- Prior experience in home & community care, assisted living, nursing homes, or hospice care is preferred
- Knowledge of Alzheimer’s, dementia, memory care,
- Ability to perform heavy lifting safely when required; familiarity with assistive devices like Hoyer lifts is beneficial
- Excellent communication skills for working effectively with clients, families, and healthcare teams
- Ability to work independently while following detailed care plans and protocols
- Basic cooking skills for meal preparation aligned with dietary restrictions or preferences
