Overview

The Operations Care Coordinator serves as the central point of contact between clients, families, field caregivers, and agency leadership. This role combines hands-on care coordination with operational oversight ensuring services are delivered safely, documentation is compliant, and the agency continues to grow through community outreach and partnership development.

Duties

  • Conduct client intakes, assessments, and in-home evaluations
  • Develop, implement, and monitor individualized care plans
  • Match caregiver qualifications and availability to client needs
  • Coordinate caregiver introductions and client onboarding
  • Serve as primary point of contact for clients and families
  • Coordinate with physicians, case managers, MCOs, and healthcare providers
  • Monitor client progress and adjust care plans as needed
  • Handle after-hours and weekend emergency calls as needed
  • Conduct home visits to assess service quality and client satisfaction

Qualifications

  • Associate's degree or equivalent; 2 years home care experience
  • Knowledge of CHC, OLTL, ODP, Medicaid/MCO billing
  • Strong organizational, communication & leadership skills
  • Proficiency with EVV systems and electronic documentation
  • Ability to manage multiple priorities under pressure
  • Experience with scheduling and care planning platforms
  • Professional presentation and community outreach skills
  • Familiarity with PROMISe provider enrollment requirements

Pay: $25.00 - $30.00 per hour
