We are seeking a Care Coordinator who will oversee the entire client case management process, including care coordination, caregiver scheduling, EVV monitoring, and supervisory visits.
This position is hybrid, combining office-based coordination with field visits to client homes.
Position Summary
The Care Coordinator is responsible for managing a caseload of clients and caregivers, ensuring that each client receives consistent, high-quality home care services.
The Care Coordinator will oversee the full lifecycle of the client case, including:
- Client intake and service setup
- Caregiver assignment and scheduling
- Electronic Visit Verification (EVV) monitoring
- Supervisory visits
- Documentation and regulatory compliance
This role also includes EVV scheduling responsibilities, ensuring all caregiver shifts are properly scheduled, verified, and documented in the agency’s EVV system in compliance with Pennsylvania home care regulations.

Pay
$15 – $17 per hour starting (based on experience)
Work Schedule
- Monday – Friday
- Some evenings or weekends may be required depending on client needs

Key Responsibilities Client Case Management
The Care Coordinator will oversee the entire care case for assigned clients, including:
- Managing a caseload of clients receiving home care services
- Conducting client intake and service coordination
- Developing and maintaining individualized care plans
- Monitoring client satisfaction and service quality
- Communicating with families regarding client needs and updates
- Responding to service issues or concerns
EVV Monitoring & Compliance
The Care Coordinator will monitor and manage caregiver visits through the Electronic Visit Verification (EVV) system, including:
- Monitoring caregiver clock-ins and clock-outs
- Reviewing EVV visit records for accuracy
- Ensuring all scheduled visits are completed
- Addressing missed or late visits
- Verifying caregiver documentation
- Ensuring compliance with EVV requirements for Medicaid and managed care organizations
- Following up with caregivers regarding visit verification issues
Scheduling & EVV Scheduler Duties
The Care Coordinator will also perform EVV scheduler responsibilities, including:
- Creating and maintaining caregiver schedules
- Assigning caregivers to client shifts
- Coordinating shift coverage for call-offs
- Managing last-minute scheduling changes
- Ensuring all shifts are properly entered into the EVV system
- Communicating schedules to caregivers and clients
- Maintaining continuity of care by assigning appropriate caregivers
Field Visits & Supervisory Oversight
- Conduct supervisory visits in client homes
- Perform caregiver performance evaluations
- Ensure services are delivered according to the care plan
- Identify changes in client condition or service needs
- Provide caregiver guidance and support
Caregiver Coordination
- Maintain communication with caregivers regarding schedules and assignments
- Assist with caregiver onboarding coordination
- Monitor caregiver performance and reliability
- Address caregiver concerns and service issues
Compliance & Documentation
- Maintain accurate client records and documentation
- Ensure compliance with Pennsylvania Department of Health home care regulations
- Document supervisory visits and care updates
- Maintain HIPAA confidentiality standards
- Ensure all case documentation meets agency compliance requirements
Qualifications
Preferred candidates will have:
- Experience in home care coordination, scheduling, or case management
- Knowledge of EVV systems used in home care
- Strong organizational and multitasking skills
- Excellent communication and problem-solving abilities
- Ability to manage multiple client cases simultaneously
- Ability to work independently and as part of a team
Education & Experience
Preferred:
- Associate’s or Bachelor’s degree in healthcare, social services, or related field
OR
- Minimum 1–2 years experience in home care scheduling, EVV coordination, or management
Requirements
- Valid driver’s license and reliable transportation for field visits
- Ability to travel within Philadelphia and surrounding counties
- Ability to pass background checks required by Pennsylvania home care regulations
- Must complete all required agency training and compliance programs
- Ability to maintain client confidentiality in accordance with HIPAA standards
