Director Patient Care -Home Health Industry

Director of Patient Care Services needed in Monterey CA (Carmel area) 93940. We will provide relocation services. Company is a leader in the Home Health industry, very reputable and established company. They have a great benefit package and 2 annual bonuses.

The Director of Patient Care Services (DPCS) in Home Health provides oversight and support of all patient care and associated personnel for assigned program. This oversight includes patient and personnel assignments, coordination of care, coordination of referrals, assessment of patient needs, and ensuring the plan of care is developed, implemented and updated. The DPCS is responsible for the services provided by the home health and hospice programs and strives to provide the highest quality of care through all service lines.

Responsibilities

The DPCS is responsible for their assigned program(s) lines of service and overall results compared to goals. They will effectively ensure the facilitation of required Interdisciplinary Group Meetings, Case Conferencing, and Coordination of Care. They will investigate and report any problem relating to patient care or conditions which might harm the patient and/or employee well-being following appropriate processes.

Responsible for recruiting, hiring, evaluating and terminating personnel in collaboration with Human Resources. Directs the planning and completion of orientation for new employees.

Participates in the Program’s ongoing Quality Assessment and Performance Improvement (QAPI) program.

The DPCS is a key member of the leadership team and works closely with other leaders to coordinate internal services with the services of other community agencies, to increase our presence in our service area and to ensure a positive and healthy working environment.

The DPCS will primarily be focused on internal marketing with respect to the excellence of the clinical program, great customer service, and expansion of clinical programs

The DPCS outreach goals shall include continued active relationships with providers and community activities that provide opportunities to grow census.

The Director of Patient Care Services ensures compliance with state, federal, and accreditation regulations and guidelines in accordance with company policy. Plans for and ensure compliance with policies and procedures which meet Medicare and Medicaid guidelines

Education/Experience/Skills/Qualifications

Possession of a current RN license to practice in the state of California and current CPR certification. Three year’s experience as a Home Health Supervisor required. Ability to exercise independent judgment and critical thinking to supervise and coordinate clinical professionals and agency support staff.
