Job Summary:

The Physical Therapist provides skilled physical therapy services to patients in their homes, helping them regain mobility, improve strength, and increase independence following illness, injury, surgery, or hospitalization. This role involves evaluating patient needs, developing personalized care plans, and working closely with interdisciplinary healthcare teams to deliver high-quality, patient-centered care in a home environment.

Key Responsibilities:

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans based on diagnosis, physician referrals, and patient goals.

• Deliver therapeutic interventions including manual therapy, therapeutic exercise, neuromuscular re-education, gait training, and modalities (e.g., ultrasound, TENS).

• Treat a diverse caseload including orthopedic, neurological, post-operative, sports injury, and chronic pain conditions.

• Educate patients and families on treatment goals, home exercise programs, injury prevention, and condition management.

• Document evaluations, progress notes, daily treatments, and discharge summaries accurately and in compliance with hospital and regulatory standards.

• Collaborate with physicians, nurses, occupational therapists, and other care team members for coordinated patient care.

• Maintain a clean, safe, and well-organized treatment space in accordance with hospital infection control protocols.

• Participate in departmental meetings, quality improvement initiatives, and professional development activities.

Qualifications:

• Doctor of Physical Therapy (DPT) or Master’s degree from an accredited Physical Therapy program.

• Current state licensure as a Physical Therapist.

• Experience in an outpatient or hospital setting preferred.

• Excellent communication, clinical reasoning, and documentation skills.

• Ability to manage a full and diverse outpatient caseload.
