We are currently seeking a motivated and compassionate Physical Therapist (PT) to join a dynamic outpatient rehabilitation team. This role offers the opportunity to work with a diverse patient population while delivering high-quality, patient-centered care in a modern clinical environment.

Position Summary

The Physical Therapist will evaluate patients, develop individualized treatment plans, and implement evidence-based interventions to improve mobility, reduce pain, and restore functional independence. The ideal candidate is passionate about clinical excellence and committed to helping patients achieve optimal recovery outcomes.

Key Responsibilities

• Perform comprehensive evaluations to assess patients’ functional status and rehabilitation needs.
• Develop and implement individualized, goal-oriented treatment plans.
• Provide evidence-based interventions, including manual therapy, therapeutic exercise, neuromuscular re-education, and patient education.
• Document evaluations, progress notes, and discharge summaries in compliance with clinic, payer, and regulatory requirements.
• Collaborate with physicians, therapists, and support staff to ensure coordinated and effective patient care.
• Monitor patient progress and adjust treatment plans as needed to maximize outcomes.
• Maintain compliance with state licensure requirements, professional standards, and clinic policies.

Qualifications

Required

• Doctor of Physical Therapy (DPT) or equivalent degree from an accredited program.
• Current or eligible state Physical Therapy license.
• Strong communication, interpersonal, and clinical reasoning skills.
• Commitment to delivering patient-centered, evidence-based care.

Preferred

• Previous outpatient orthopedic or clinical experience.
• Familiarity with EMR documentation systems and outcome tracking tools.
