We are currently seeking a full-time Physical Therapist (PT) to join a skilled nursing and assisted living rehabilitation team. Key Responsibilities
- Perform comprehensive evaluations of residents to assess strength, mobility, balance, coordination, and functional ability.
- Develop and implement individualized physical therapy treatment plans based on clinical findings and physician orders.
- Provide evidence-based therapeutic interventions including therapeutic exercise, gait training, manual therapy, pain management techniques, and modality use.
- Document evaluations, treatments, and resident progress in compliance with regulatory, facility, and professional standards.
- Collaborate with nurses, physicians, occupational therapists, speech-language pathologists, and other healthcare team members to ensure coordinated care.
- Communicate effectively with residents and families regarding treatment plans, progress, and goals of care.
- Ensure compliance with all federal, state, and facility policies, including HIPAA, Medicare/Medicaid documentation guidelines, and infection control protocols.
- Participate in interdisciplinary care planning meetings and facility in-services as needed.
Qualifications
Required
- Graduate of an accredited Physical Therapy program (Bachelor’s Degree required; Master’s Degree preferred).
- Current or eligible state Physical Therapy license.
- Strong communication, clinical reasoning, and documentation skills.
