Nurse Practitioner

Job Summary:

• Provide high-quality patient care in accordance with state Nurse Practice Act and established standards.
• Implement and coordinate patient care plans under the supervision of registered nurses.
• Conduct thorough assessments and educate patients on self-care techniques and disease management.

Key Responsibilities:

• Deliver direct patient care services in a home health setting.
• Collaborate with interdisciplinary teams to ensure seamless patient care.
• Document accurate and timely information to support patient care plans.

Requirements:

• Current active license as an LPN in Massachusetts.
• Graduate of an accredited practical nurse program.
• Minimum 3 years of nursing experience, preferably in community or home health settings.
