⚕️ Your Impactful Role As An LPN
As a Licensed Practical Nurse with Loyal Home Care Services, you'll be at the center of personalized care plans - delivering top-tier support and forming trusted relationships with clients and families.
Responsibilities
- Deliver skilled nursing care based on the Plan of Care and within scope of MA Nurse Practice Act
- Support and carry out care plans initiated by the Registered Nurse
- Provide education and guidance to patients and families on condition management and at-home care
- Monitor health status, document accurately, and report updates to the clinical team
- Collaborate with RNs and coordinate additional services (PT, OT, HHA, MSW, etc.)
- Use medical equipment safely, maintaining infection control standards
- Stay current on best practices through ongoing clinical development
- Be an advocate and source of encouragement for your patients
- Active LPN license in Massachusetts
- Graduate of an accredited LPN or vocational nursing program
- Minimum of 3 years nursing experience (home health or med/surg preferred)
- Current CPR/BLS certification
- Reliable transportation and valid driver's license
- Up-to-date health certificate & TB test
- Fluent in English (spoken and written)
- Demonstrated knowledge of HIPAA & patient confidentiality
- Must be 18 years or older and authorized to work in the U.S.
