CareConnect is seeking compassionate and dedicated Licensed Practical Nurses (LPNs) to provide high-quality patient care in various locations throughout Worcester County.
As an LPN with CareConnect, you will have the opportunity to work closely with our experienced team of healthcare professionals to deliver personalized care that meets the unique needs of each patient.
- Duties:
- Directly care for patients in accordance with the state Nurse Practice Act.
- Implement patient care plans developed by registered nurses.
- Accurately document patient progress and adjust care plans as needed.
- Educate patients and their families/caregivers on self-care techniques and disease management strategies.
- Coordinate with other healthcare professionals to ensure comprehensive patient care.
- Prepare and use medical equipment and supplies while maintaining asepsis standards.
- Stay up-to-date on clinical best practices through ongoing education and training.
- Perform additional tasks as assigned to support patient care and team collaboration.
Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program.
- Current, active LPN licensure in Massachusetts (MA).
- Up-to-date CPR/BLS certification.
- A minimum of three years of nursing experience, preferably in community/home health or medical/surgical settings.
- Valid state driver's license and reliable automobile.
- Authorized to work in the U.S. and able to provide proof of eligibility.
- Fluent in English, both spoken and written.
- Strong understanding of HIPAA regulations and commitment to patient confidentiality.
- Current health certificate/physical examination and TB testing results.
- At least 18 years old.
