Job Title: Licensed Practical Nurse
We are seeking compassionate and skilled Licensed Practical Nurses to provide exceptional care to patients in their homes. As a key member of our team, you will be at the heart of our client's care.
Duties include:
- Providing direct patient care in accordance with the state Nurse Practice Act.
- Implementing the plan of care initiated by the registered nurse.
- Maintaining accurate and timely documentation that aligns with the patient's plan of care.
- Educating patients and their families/caregivers on self-care techniques and relevant information related to the patient's diagnosis and care plan.
- Coordinating home health services by communicating identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
- Using and preparing medical equipment and supplies effectively, ensuring adherence to aseptic techniques.
- Participating in personal and professional growth and development activities to enhance clinical skills and knowledge.
- Performing additional duties as assigned by the registered nurse to support patient care and team collaboration.
Qualifications:
- Graduate of an accredited practical nurse or vocational nursing program.
- Current, active license as an LPN in Massachusetts.
- Up-to-date CPR/BLS certification.
- Minimum three years of nursing experience, preferably in community/home health or medical/surgical settings.
- Valid state driver's license and reliable automobile.
- Authorized to work in the U.S. and proof of eligibility if necessary.
- Fluent in English, both spoken and written, to communicate effectively with patients and the healthcare team.
- Strong understanding of HIPAA regulations and commitment to maintaining patient confidentiality.
- Current health certificate/physical examination and TB testing results.
- At least 18 years of age.
