Responsibilities
- Develop and execute a strategic community partnership plan to generate new client referrals and increase census growth
- Identify and cultivate new referral relationships that turn into consistent business
- Meet and exceed monthly and quarterly growth targets
- Establish and grow partnerships with:
- Home Care, Home Health, and Hospice providers
- Hospitals, physician offices, and discharge planners
- Assisted Living and Senior Living communities
- VA programs, senior centers, and local community organizations
- Maintain strong relationships with referral partners, clients, and families
- Serve as a trusted liaison and resource within the healthcare and senior care community
- Ensure a smooth onboarding and handoff experience for new clients
- Represent CareBuilders at Home at networking events, community meetings, and health fairs
- Increase brand awareness and position our agency as a trusted and preferred provider
- Monitor marketing activity, referrals, and conversions
- Provide regular updates on pipeline growth and partnership activity
- Collaborate closely with operations and care teams to ensure excellent service delivery

- Organized, persistent, and proactive in building opportunities
- 2 years of healthcare sales, marketing, or business development
- Experience in Home Care, Home Health, Hospice, or senior care services
- Established relationships within the local healthcare or senior care community is a strong plus
- Strong communication, networking, and presentation skills
- Experience using CRM or referral tracking systems preferred
- Valid driver’s license and reliable transportation
- Ability to travel locally throughout Bucks County and surrounding areas

Salary : $60,000 - $75,000
