We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team at Carease Health - TX in Houston, TX. As a Social Worker Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities.

Job Summary

As a Social Worker Care Companion, you will work closely with our experienced clinical team to deliver comprehensive care to our patients. This is a part-time opportunity with a flexible schedule, approximately 9 hours per week, Monday-Friday, within regular 8-5 hours. We offer paid training, constant support and guidance from our clinical team, and a competitive flat salary ranging from $12,000 to $14,000 annually.

Key Responsibilities

• Visit designated facilities in accordance with the schedule set with your supervisor
• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
  
  
  

Qualifications

• Associate Degree in Social Work, Psychology, Counseling, or another mental health-related field
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
• Strong communication, documentation, and relationship-building skills
