Carease Health - NC is seeking a dedicated Social Worker/Care Companion to join our Collaborative Care team. As a Social Worker/Care Companion, you will provide emotional support, brief therapeutic interventions, and mental health assessments to residents in skilled nursing facilities. This part-time role is ideal for professionals who want to make a direct impact on the lives of older adults, helping them feel heard, supported, and emotionally connected within their environment.
Key Responsibilities:
- Visit designated facilities in accordance with the schedule set with your supervisor.
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs.
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns.
- Document visit notes and maintain detailed records of interactions and interventions after each visit.
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP.
- Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field.
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
- Strong communication, documentation, and relationship-building skills.
- Education: Associate Degree
- Experience: 1 year
