Key Responsibilities:
- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
- Associate's, Bachelor's, or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- 1 year of experience
- Education: Associate Degree
- Meaningful, resident-focused work in a supportive team environment
- Regular supervision and collaboration with psychiatric providers
