Responsibilities:
- Build strong relationships with the facility and maintain a patient-centered approach
- Conduct on-site visits and provide remote care to residents
- Document patient visits and maintain accurate records
- Collaborate with other healthcare professionals to provide comprehensive care
- Participate in Continuous Quality Improvement (CQI) initiatives to enhance patient outcomes
- Active NP license in the state of practice
- 1 year of experience in Geriatric care and documentation review
- Excellent communication, collaboration, and teamwork skills
- Masters degree in Nursing
