This is a full-time, on-site role for a Social Worker/Care Companion based in Lakewood, NJ. The Social Worker/Care Companion will provide empathetic and hands-on support to residents and staff at Skilled Nursing Facilities. Responsibilities include conducting mental health check-ins, offering emotional support, assisting with care coordination, and advocating for residents' well-being. Additionally, the role involves collaborating with medical staff, facilitating communication between residents and families, and ensuring compliance with care guidelines.
- Experience in social work, counseling, or care coordination.
- Empathy, active listening, and strong interpersonal communication skills.
- Ability to network and collaborate effectively with healthcare teams, residents, and families.
- Proficiency in problem-solving and conflict resolution to address resident needs and concerns.
- Knowledge of mental health support practices and patient advocacy.
- Understanding of healthcare compliance and chronic care management processes, such as RPM and CoCM, is a plus.
- Bachelor’s degree in Social Work, Psychology, or a related field (Master’s degree preferred).
- Experience working in a Skilled Nursing Facility or within elderly care is highly desirable.
