Social Work/Care Companion
Job Summary:
Join Carease Health - IL's Collaborative Care team as a compassionate and dedicated Mental Health Provider, making a direct impact on the lives of older adults in skilled nursing facilities. As a Social Work/Care Companion, you'll provide emotional support, brief therapeutic interventions, and mental health assessments to residents, helping them feel heard, supported, and emotionally connected within their environment.
Key Responsibilities:
- Visit designated facilities in accordance with the schedule set with your supervisor
- Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
- Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
- Document visit notes and maintain detailed records of interactions and interventions after each visit
- Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Qualifications:
- Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health–related field
- Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
- Strong communication, documentation, and relationship-building skills
- Education: Associate Degree
- Experience: 1 year
